Abstract
Objetive: Giant Cell Tumors (GCT) of the bone represent 20% of all benign bone tumors. Although they are locally aggressive, the occurrence of metastases is rare, and the standard treatment for GCT is surgery. In 2013, Denosumab therapy was approved for unresectable cases or those with a high risk of morbidity following surgical resection. This study aimed to evaluate the efficacy of neoadjuvant therapy with Denosumab in patients with GCT who underwent wide resection at the National Institute of Traumatology and Orthopedics (INTO). A retrospective and analytical study was conducted with 51 patients diagnosed with bone GCT, treated between January 2013 and December 2021. The patients were divided into two groups: 1) "Conventional" group (n=35), which included patients who underwent only surgical resection; 2) "Denosumab" group (n=16), which included patients treated with Denosumab before surgical resection. Both groups excluded patients with metastases or recurrence prior to treatment and had a minimum post-surgical follow-up of six months. Patients subjected to intralesional surgery were excluded from the study. Of the 51 patients evaluated, 54.9% were female and 70.5% were in the age range of 15 to 39 years. From the Denosumab group, 18.8% experienced adverse events, with alopecia and aseptic osteonecrosis of the jaw reported by two patients each, and nausea reported by one patient. In total, 5 patients developed local recurrence of the lesion, with 3 from the Denosumab group (3/16; 18.8%) and 2 from the Conventional group (2/35; 5.7%). Four patients developed pulmonary metastasis, where 2 patients belonged to the Denosumab group (2/16; 12.5%) and 2 patients belonged to the Conventional group (2/35; 5.7%). In the radiological response to the medication, 50% of the patients experienced a reduction in tumor size, and 75% showed the formation of a sclerosis halo. In the histological analysis, it was found that the majority (13/16; 81.3%) presented a prevalence of stromal cells, with bone neoformation in 100% of the cases and the presence of fibrosis and/or hyalinization in 93.8% (15/16) of the cases. Both histological and radiographic responses did not demonstrate statistical association with clinical outcomes. The limitations of the study include the small sample size and being restricted to a single center. In conclusion, this work provides valuable information on the treatment of GCT with Denosumab, despite the mentioned limitations, suggesting that further studies be conducted for the clinical application of the findings and for planning future research.
