Abstract

Objective: The valgus deformity of the tibia is not something common in the practice of the orthopedic doctor. The reestablishment of the mechanical axis of the lower limb is necessary to avoid the early degenerative process of the joints of the lower limbs. There is no consensus in the literature on the ideal correction method. For this, individualized clinical and radiological evaluation is necessary, performing surgical planning. The aim of this study was to systematize the technique of gradual correction of valgus deformity of the tibia. The study included patients from the Unified Health System who used an external fixator device with monolateral mounting on the tibia between January 2012 and May 2022 of the National Institute of Traumatology and Orthopedics of Rio de Janeiro (INTO). The collected data were compared to the results with those in the literature. 24 valgus deformities of the tibia in 21 patients were included in this study, all of whom underwent proposed surgery, by indication of an orthopedist from the Center for Specialized Care for the Treatment of Dysmetria and Deformity of the Locomotive System. The data collected from the physical records were age, color, gender, laterality, date of surgery, date of hospitalization, date of hospital discharge, date of the guide of the removal of the external fixator, complications observed in outpatient medical evolutions, body mass index (BMI), Classification of the American Society of Anesthesia (ASA) and comorbidities. In the radiographs presented in the radiology system, the pre- and postoperative angles of the tibial deformities were measured using the Mdicomview® software. In the sample, a predominance of males was observed in 54.2% of the cases, the mean age presented was 30.8 ± 15.9 years. The most frequent etiology was congenital in 14 cases (58.3%) with apex of the deformity located in the middle third (79.2%), non-white population in 70% of cases. Comorbidities were present in 45.8% and mean BMI of 26.1 ± 5.5 kg/m2. The length of stay ranged from 2 to 13 days, with a mean of 3.1 ± 2.6 days. The final sample presented deformity of 14.7 ± 6.4 degrees. The proximal angle of the tibia had a mean correction of 5.2 ± 6.1 degrees and the distal lateral angle of the tibia had a mean correction of 7.2 ± 9.1 degrees. The total fixator time ranged from 73 to 229 days with a mean of 149.7 ± 36.1 days. There was complication in eight cases. The most frequent complication was pin path infection in five cases, two cases with medial cortical fracture and one case with fibular nerve neuropraxia. The results of this study suggest that the gradual correction with the monolateral assembly of the external fixator presents satisfactory deformity correction potential and with complications compatible with the literature data.
