Abstract

Objetivo: Introdução: A ruptura do ligamento cruzado anterior (LCA) é a lesão ortopédica mais frequente no joelho, acometendo principalmente jovens fisicamente ativos e atletas. Objective: Introduction: Anterior cruciate ligament (ACL) rupture is the most frequent orthopedic injury to the knee, affecting mainly physically active young people and athletes. ACL injury can lead to changes in maximum strength, in the rate of torque development (RTD), which is the muscle's capacity to generate explosive strength, and electromechanical delay (EMD), which is the time between the detection of muscle activation and the production of torque in the joint. Objective: To study the effects of ACL rupture on the capacity to produce maximum and explosive strength in the quadriceps, the ability to perform voluntarily muscle recruitment and to calculate the electromechanical delay. Methods: Twenty-eight individuals aged between 18 and 40 years and diagnosed with unilateral ACL rupture were in the pre-surgical phase. Electrodes were placed over the vastus lateralis, vastus medialis and rectus femoris to acquire the electromyographic signal. An isokinetic test was performed to define the peak torque of the extensor and flexor muscles of both legs and the optimal angle. Afterwards, an explosive isometric test was performed to calculate RTD and EMD. The test consisted of three explosive repetitions, where the individual was instructed to apply force to extend the knee as quickly as possible from rest. Results: After ACL rupture, we found a significant difference between the limbs p <0.001 in maximum strength the knee extension and flexion, in RTD50 p = 0.009 and in RTD250 p = 0,003. No differences were found in the normalized RTD nRDT50 p = 0.385 and nRDT250 p = 0.755. Regarding AEM, we did not find any significant difference between the limbs, however we found a decrease in muscle recruitment in the CTL limb p < 0.001 and in the ACL limb p = 0.001. Conclusion: The results of this study suggest that after ACL rupture there is a significant reduction in the maximum and explosive strength of the involved limb, when compared to the healthy contralateral limb of the study participants.
