Objetive: The anterior cruciate ligament (ACL) is one of the most important structures in the knee, playing a key role in its stability and proprioception. Injury to the ACL triggers ligament laxity, leading to an inability to perform sports activities or even daily activities, especially in rotational movements, The anterolateral ligament, which plays an important role in the anterolateral stability of the knee, and the posterior slope of the proximal surface of the tibia (tibial slope) seem to influence tibial anteriorization. The objective of this study was to assessment through magnetic resonance imaging, the anterior translation of the tibia in patients with and without ACL rupture and in patients undergoing ACL reconstruction, and to analyze the influence of tibial slope and anterolateral ligament in this process. In this study, seventy-seven knee magnetic resonance images of the knee from the D'OR Institute for Research and Teaching database were assessment. The exams were divided into four groups: without ACL rupture, with ACL rupture, with satisfactory ACL reconstruction and with ACL reconstruction failure. The results evidenced greater anterior translation of the tibia on the medial tibial plateau in knees with ACL graft failure (5.8 ± 2.62 mm), followed by the group with intact ACL graft (2.4 ± 3 mm), and the group with ACL tear (2.1 ± 2.5 mm) compared to the group without ACL injury (-0.9 ± 2 mm). The lateral tibial slope was higher in the group with ACL graft failure and in the group with intact ACL graft compared to the control group. Anterior translation of the lateral tibial plateau was more pronounced in the group with ACL rupture than in the others groups. The results of this study demonstrated an increased in anterior tibial medial plateau translation and lateral tibial slope in the group of patients who failed the ACL reconstruction in relation to the control group. The visualization of the anterolateral ligament did not cause statistical significance between the analyzed groups The analysis of the variables studied in this work is important to avoid possible failures in the reconstruction of anterior cruciate ligament.
