Objectives: To study the tibial nerve bifurcation and its relationship to the tarsal tunnel, medial malleolus, and the most frequent ramification patterns of calcaneal branches. Methods: After dissection of thirty legs, the tibial nerve was identified and isolated. Four references were employed for the study of tibial nerve ramification: 1, the axis between the medial malleolus and the calcaneal attachment (CMA, calcaneusmalleolar axis); 2, flexor retinaculum; 3, medial malleolus apex; and 4, the upper edge of the abductor hallucis muscle. Calcaneal branches were grouped to establish the identified patterns. Results: Tibial nerve bifurcation occurred within the tarsal tunnel in 26 limbs (86.7%), a proximal situation occurred in three cases (10%), and a distal situation in one case (3.3%). Regarding CMA, branching coincided with the axis in 10 samples; in 10 samples the branching was proximal; and in the remaining 10, branching occurred distally. Regarding the medial malleolus apex, bifurcation was proximal in 29 samples. A number of two branches appeared more frequently (n = 14). Calcaneal branching at the retinaculum occurred in 27 cadavers; in 18 cadavers, branching occurred proximally to the retinaculum. No calcaneal branching distally to the retinaculum were observed. Conclusions: Tibial nerve bifurcation occurs more often inside the tunnel. A single branch emission inside the tunnel, or a proximal branch from the tibial nerve, and a second branch from the lateral plantar nerve are the most frequent patterns. The more distal is the calcaneal branch origin, the more frequent is the origin from the lateral plantar nerve.
