Abstract

Objetive: Traumatic brachial plexus injury (TBPI) limits movement and sensitivity of the upper limb. Surgery is the main treatment. The pattern of injury and the possibilities for surgical intervention vary considerably, therefore a sensorimotor assessment of the entire upper limb is recommended. The aims of this study were to investigate the sociodemographic and sensorimotor profile of patients with TBPI, and to propose a new method for classifying the injury profiles of the participants, using the k-means clustering algorithm in the strength values of the entire injured limb. Data was extracted from the LabNeR-UFRJ TBPI database. Sixty-three participants were included with a mean age of 34.5 ± 11.1 years, predominantly male (84.1%), with black or brown skin (69.8%). The main cause of the injury was the motorcycle accident (76.2%), complete injury was the most recurrent (44.4%). Of the surgical procedures performed, 78.4% were neurotizations, followed by 16.5% of orthopedic surgeries. When assessing the grade force of the upper limb, the shoulder was the most affected region, with the strength grade ≤ 2 (does not overcome gravity) for flexion, external rotation and abduction (71.5%, 71.4% and 68.3% respectively). The elbow evaluation revealed strength grades ≤ 2 in 54% of the participants for flexion and 49.2% for extension. The passive range of motion was different from the reference values for all upper limbs (p<0.001), except for wrist flexion (p=0.2). Tactile and painful superficial sensitivity was more affected in C5 and C6 dermatomes (> 80% of the sample). For deep sensitivity kinesthesia and proprioception, the most affected region was the index finger (> 27%). For pallesthesia, the most affected region was the elbow (60.3%). By applying the K-means algorithm to the strength values, it was possible to split the sample into 3 groups (group 1 - n=27 – moderate proximal deficit; group 2 – n=13 – mild global deficit; group 3 – n=23 – severe global deficit). There was also a difference between the 3 groups for superficial sensitivity, with the group 3 being the most compromised and the group 2 the least. Group 3 had a higher percentage of complete lesions (91%), pain (87%) and participants undergoing surgery (96%). The study proposed an unprecedented way of classifying the BPI, demonstrating that the greatest motor deficits are related to the most serious injuries, despite of adherence to rehabilitation and/or the surgical procedures.
