Abstract

Objective: The rupture of the quadriceps tendon is uncommon and disabling. Complete lesions of this tendon must be treated surgically. The treatment of chronic quadriceps injuries is still a challenge for the surgeon. The objective of this study was to evaluate the clinical, functional and isokinetic results of a new technique for the reconstruction of chronic quadriceps injuries, called "Quadriceps Reconstruction with Modified Pulvertaft Weave Technique". Range of motion (ROM) of the knee and clinical scores of Visual Analog Pain Scale (VAS), Lysholm and International Knee Documentation Comitte Subjective Knee Form (IKDC) were evaluated. Isokinetic assessment was performed in the postoperative period, through isokinetic analyzes between the operated and the non-operated (control) limb. Nine patients with 10 chronic quadriceps injuries (mean injury time 23.6 months) were evaluated. The extension deficit in the preoperative period was 46.5 ° ± 10.0 °, significantly reducing in the postoperative period, with all subjects obtained 0 ° (p <0.001). The knee ROM in the preoperative period was 79.5 ± 14.2 °, significantly increasing in the postoperative period to 109.0 ± 15.4 ° (p <0.001). The VAS in the preoperative period 2.3 ± 2.4 points and in the postoperative period it was 2 ± 2 points; the preoperative Lysholm 55.7 ± 14.3 points and in the postoperative it was 65.6 ± 19.1 points. The preoperative IKDC was 30.3 ± 9.9 points and in the postoperative it was 43.6 ± 12.3 points. No clinical score was statistically significant. Significant differences were found for Peak Extension Torque, Maximum Isometric Voluntary Contraction and Rate of force development (RFD)250. The control member presented such measures superior to the operated one. However, no statistically significant differences were found between the limbs in the postoperative evaluation in the initial RFDs (0 to 200 milliseconds). Noting that muscle strength was reestablished on the operated side in a similar way to the control side in these time bands. Concluding that the evaluated technique was able to reestablish the active extension of the operated limb in all patients, and that the muscular strength was reestablished in the periods of strength necessary for the accomplishment of the majority of the daily activities (early RFDs).
